BEAVERTON — Providence ElderPlace is expanding its services to Washington County. The new facility in Beaverton is Providence ElderPlace’s eighth location, and the first outside of Multnomah County. The health and social center opened Sept. 1.
Providence ElderPlace provides medical and social services to help keep frail, older individuals out of nursing homes and living in their community. The program includes health, housing, social service and care coordination for older adults and peace of mind for families.
Providence ElderPlace serves individuals living in their own homes, with family members, or in contracted housing.
