Dr. David Cook died July 20. He was 75.

Dr. Cook was born Dec. 4, 1937, in Chicago. He graduated from Notre Dame University in 1959 and from St. Louis University Medical School in 1963, where he met his wife Marie. He served in the U.S. Army for two years. After completing a residency at Cook County Hospital in Chicago and a residency at the Mayo Clinic, he and Marie moved to Portland in 1969. Dr. Cook worked at OHSU for 43 years as an endocrinologist.

He is survived by his wife Marie; children, Karen, Michael, Paul and Melanie; sons-in-law Sam and Josh; five grandchildren; brother, Richard; and sister, Carol.

A memorial Mass will be celebrated at St. Cecilia Church in Beaverton, on Tuesday, July 30, at 4 p.m. A rosary will precede the Mass at 3:30. A reception will be held after the Mass at the parish center. Memorial donations can be made to St. Cecilia Adopt-a-Student program or LifeWorks Northwest.
